I am writing to inform you that I will no longer be practicing at Mango Medical Clinic effective November 15, 2023 as I will be relocating to Divine Medical Clinic in North Burnaby. I will be available for all of my patients at this new location.

Should you choose to remain in my care at my new practice location, you can simply start calling our new number to see me at the new clinic, starting in November 2023. As you are my patient, your medical record will be transferred to Divine Medical Clinic by default at no cost to you unless you inform us not to do so.
If you do not want to remain in my care, please take the steps to find another family physician to continue your care as soon as possible. As you are attached to me, and not Mango Medical Clinic, you will become unattached and can no longer seek care at Mango Medical Clinic after November 15, 2023. You may contact your local Division of Family Practice (e.g.: if you live in Burnaby you can get in touch with Burnaby Division of Family Practice) for assistance in finding another physician. You can also go on websites such as https://bccfp.bc.ca/for-the-public/find-a-family-doctor/ for more resources.
If any lab/imaging results are obtained after Nov 15/2023, I would encourage you to redirect the result to your new family doctor. If you are unable to find another doctor by that time, book an appointment to see me as a Walk-In physician at Divine Medical Clinic to review the results until you find a new family doctor. My colleagues at Mango Clinic will NOT be able to see you or review your results with you. If you need standing orders, please get a new doctor to give you a new requisition as I may not be able to follow up on those results in the future.
As per provincial regulations, a copy of your medical records will remain intact at Mango Medical Clinic but it will not be updated after my departure. Once you find a new provider you can send a written medical record transfer request form that requires your signature and identification of the physician selected. A full copy of your records will be transferred to your new physician at the usual record transfer cost.
Please note that there is NO change in my practice until November 15, 2023.
