Dear patient,
This letter is to inform you that I will be relocating my medical practice from Mango Medical Clinic to J-Von Family Medical Centre in April 2024. The new clinic address is 4468 Beresford Street which is 160 meters directly east of Mango Medical Clinic. The new clinic telephone number is 778-379-8299.
My last day of work at Mango Medical Clinic will be Thursday April 11, 2024. Starting on Tuesday April 16, 2024, I will be working two days a week on Tuesdays and Fridays at J-Von Family Medical Centre.
All of my current family practice patients will be able to continue to see me at the new location. I will arrange for your patient chart records to be transferred to the new clinic at no cost to you. If you do not wish to follow me to the new clinic or have your chart transferred, please inform me by contacting Mango Medical Clinic at your earlier convenience. Please refrain from contacting J-Von Family Medical Centre prior to April 2024, as I will still be treating you at my current location until April 11, 2024.
