I am writing to inform you that I will no longer be doing family practice at Mango Medical Clinic effective September 30/2021 as I will be relocating and transitioning to a different path in my career. I will be available until that time for your health care needs.
Please find another family physician to continue your care. You may contact your local Division of Family Practice (ex: if you live in Burnaby you can get in touch with Burnaby Division of Family Practice) for assistance in finding another physician. You can also go on websites such as https://bccfp.bc.ca/for-the-public/find-a-family-doctor/ for more resources.
If any lab/imaging results are obtained after Sept 30/2021, I would encourage you to redirect the result to your new family doctor. If you are unable to find another doctor by that time, I will review the labs and my colleagues at Mango Clinic to review them with you. If you need standing orders, please get a new doctor to give you a new requisition as I may not be able to follow up on those results in the future.
Your medical records will remain intact at Mango Medical Clinic until you transfer your care to a new family doctor. You can continue to see walk in doctors at Mango Medical who will have access to your records. Once you find a new provider you can send a written medical record request form that requires your signature and identification of the physician selected. A full copy of your records will be transferred to your new physician at the usual record transfer cost.
